The Brigham and Women’s Diabetes Program provides a full range of diabetes services, including specialized outpatient medical management and comprehensive diabetes self-management education. Our clinical staff maintains close communication with our patients’ primary care physicians to ensure that care is well coordinated. Our team-based care includes a close working relationship with collaborating dietitians, clinical pharmacists and other specialists within the Brigham Health system who provide care for patients with diabetes.
