The Faulkner Advanced Clinician Team (FACT) service is a collaborative staffing model of both attending physicians and staff APP hospitalists working closely together.
We deliver high-quality care, promote quality and safety, identify and support innovations in care, and educate medical and APP students and fellows.
What we do
For more than 10 years, the FACT service members have worked together, to care for the diverse patient population at BWFH. We work closely with our clinical colleagues including: subspecialist, nursing, physical, occupational and speech therapy as well as social work and case management. We are committed to both quality of care for our individual patients and a broad focus on our community and systems of care.
To support this mission, FACT service members play a key role in professional development, medical education, and quality improvement work. Some of the notable contributions by our team include:
- Education: Development of APP grand rounds (a CME offering), which are led by a senior APP.
- Innovation: Conducting formal quality improvement work in areas such as glycemic control, health equity, interventions to address hospital acquired conditions such as CAUTIs, CLABSIs, falls, and delirium, as well as substance use disorders.
- Scholarship: Presenting posters at national conferences including the Society for hospital medicine, American Society of Addiction Medicine, and the MGB quality and safety symposium.
- Leadership: multiple FACT providers serve in leadership roles including Unit Medical directors (both MD and APP roles), committee leads, and as course directors for the PA hospital medicine fellowship.
