You may request your birth certificate from the town or city in which you lived when you were born. Or, you may request your birth certificate from the town or city where the hospital in which you were born was located. For those born at Brigham and Women's Faulkner Hospital, birth records are kept at Boston City Hall.
Please call Brigham and Women's Faulkner Hospital's Thea Stoneman Breast Imaging and Diagnostic Center at 617-983-7272 to request an appointment for a mammogram. You will need to provide information including your name, date of birth, phone number, address, and the name of the physician to whom we should send the report. If you have had films taken previously, you will be asked to bring those to your appointment.
Our Outpatient Laboratory is open Monday through Friday from 7 am to 5:30 pm (no weekends). Holiday hours may vary, so please contact the lab at 617-983-7336 or 617-983-7111 to confirm before coming in.
