James Grafton, RN, MSN, MHA, recently assumed the role of Senior Manager of Care Continuum Management at Brigham and Women’s Faulkner Hospital where he will oversee the case managers and the inpatient medical-surgical and addiction social workers and coordinate our response to the medical and psychosocial needs of these patients. He comes to BWFH from Brigham and Women’s Hospital where he most recently served as Nurse Care Coordinator.
Grafton earned a bachelor’s degree in Health Management and Policy and a master’s degree in Health Administration at the University of New Hampshire before going to work as a nursing home administrator and later going to nursing school. After several years in the long-term care field he decided to make the jump to acute care at BWH. He again went back to school, earning a master’s degree in Nursing Leadership/Health Administration at Regis College while continuing to work at BWH as a case manager in cardiac surgery.
“I’ve always enjoyed helping patients,” he says. “In my job, I help them find the services they need and act as an advocate for them. I work with all the members of the care team to create the best possible outcome for our patients.”
In his new role at BWFH, Grafton is looking forward to getting to know the Care Continuum Management staff. “Everyone has been so welcoming!” he says. “From the minute I walked through the door for orientation, I felt it. I hope that I can help others feel just as welcome. My door is always open!”
When he is not at work, Grafton enjoys spending time with friends and family. As an art collector, he also enjoys traveling and visiting museums.
